👉 ପ୍ରଥମେ ଆଧାର କାର୍ଡରେ ଥିବା ନାମ ଲେଖି, ଆଧାର ନମ୍ବର ଲେଖିବାକୁ ପଡ଼ିବ।
👉 ପରେ ସ୍ବାମୀ କିମ୍ବା ବାପାଙ୍କ ନାମ ଲେଖିପାରିବେ
👉 ଜନ୍ମ ତାରିଖ ସ୍ଥାନରେ ଆଧାର କାର୍ଡରେ ଥିବା ତାରିଖ ଦେବାକୁ ହେବ
👉 ଜୁଲାଇ ୧ ସୁଦ୍ଧା ଆବେଦନକାରୀଙ୍କୁ ଅତିକମ୍ରେ ୨୧ ବର୍ଷ ହେଉଥିବା ଦରକାର
👉 ପରେ ଜାତି ସଂପର୍କରେ ଥିବା ବକ୍ସରେ ଟିକ୍ ମାରିବାକୁ ହେବ
👉ମୋବାଇଲ ନମ୍ବର ବକ୍ସରେ ଆଧାର କାର୍ଡ ଲିଙ୍କ୍ ହୋଇଥିବା ନମ୍ବର ଦେବାକୁ ହେବ
👉 ବିକଳ୍ପ ଭାବେ ଆପଣ ଅନ୍ୟ ନମ୍ବର ବି ଦେଇପାରନ୍ତି
👉 ତାପରେ ଠିକଣା ଲେଖି, ଆଧାର ଲିଙ୍କ୍ ଥିବା ବ୍ୟାଙ୍କ ଆକାଉଣ୍ଟ ନମ୍ବର ଦେବାକୁ ହେବ
👉 ପର ପୃଷ୍ଠାରେ ଥବା ଯୋଗ୍ୟତା ମାନଦଣ୍ଡ ଆଧାରିତ ଏକ ଘୋଷଣା ପତ୍ରକୁ ନିଜେ ଆବେଦନକାରୀ ଦସ୍ତଖତ କରି ଆବେଦନ କରିବେ
👉 ଫର୍ମ ଫିଲଅପ୍ ପରେ ଏହା ସହ ନିଜର ଆଧାର କାର୍ଡ, ବ୍ୟାଙ୍କ ପାସବୁକ୍ ଓ ରାସନ କାର୍ଡର ଫଟୋକପି ଦେଇ ଅନଲାଇନରେ ସୁଭଦ୍ରା ପୋର୍ଟାଲରେ ଆବେଦନ କରିବେ
👉 ଏହାପରେ ଆବେଦନ ଗ୍ରହଣ ପତ୍ର ଆଣିବାକୁ ଯେମିତ ନ ଭୁଲନ୍ତି, ଯେଉଁଥିରେ ଆପଣଙ୍କ ଆବେଦନ ପତ୍ରର ନମ୍ବର ରହିଥିବ |
Know how to fill the Subhadra form, and what certificate to give?
- First write the name on the Aadhaar card and write the Aadhaar number.
- Later you can write the name of husband or father
- Date of birth should be given on Aadhaar card instead of date of birth
- Applicants must be at least 21 years of age as on July 1
- Then tick the box related to caste
- Aadhar card linked number should be given in the mobile number box
- Alternatively you can give any other number
- Then write the address and give the bank account number with Aadhaar link
- On the next page there will be a declaration letter based on the eligibility criteria to be signed by the applicant himself
- After filling up the form, apply online on Subhadra portal by giving photocopy of your Aadhaar card, bank passbook and ration card along with it.
- Then don't forget to bring your application receipt which will have your application number.
